CONSENT

This Disclosure and Consent Agreement applies to all fingerprinting services provided by Lake Norman TNT, LLC.

CUSTOMER RESPONSIBILITY
Customers are solely responsible for verifying fingerprint requirements with the requesting agency, employer, licensing board, government agency, educational institution, or other organization requiring fingerprints. Lake Norman TNT, LLC does not determine fingerprint requirements and cannot guarantee that fingerprints will satisfy the requirements of any requesting entity.

IDENTITY VERIFICATION
Valid government-issued photo identification is required prior to fingerprinting services. Failure to provide acceptable identification may result in cancellation of the appointment and applicable fees.

NO GUARANTEE OF ACCEPTANCE
Acceptance or rejection of fingerprints is determined solely by the receiving agency or organization. Lake Norman TNT, LLC cannot guarantee:
• Acceptance of fingerprint cards

• Acceptance of electronic fingerprints

• Approval of licenses, permits, employment, certifications, or applications

• Results of background investigations.

QUALITY OF PRINTS
Fingerprint impressions are collected using accepted industry standards and procedures. However, fingerprint quality may be affected by factors outside the control of Lake Norman TNT, LLC, including:
• Dry skin

• Worn fingerprints

• Age

• Medical conditions

• Physical labor occupations

• Scarring or injuries

• Missing digits, etc.

REPRINTS
If fingerprints are rejected by a receiving agency, a new appointment may be required. Additional fees may apply unless otherwise specified in writing.

CONFIDENTIALITY
Fingerprint information and related personal information will be handled in accordance with applicable laws and the Lake Norman TNT, LLC Privacy Policy.

LIMITATION OF LIABILITY
Lake Norman TNT, LLC shall not be responsible for delays, denials, rejections, lost opportunities, licensing decisions, employment decisions, or other consequences resulting from the acceptance or rejection of fingerprint submissions by any third party.

CONSENT
By receiving fingerprinting services from Lake Norman TNT, LLC, the customer:
• Confirms that all information provided is accurate

• Authorizes the collection of fingerprint impressions

• Acknowledges responsibility for verifying fingerprint requirements

• Understands that acceptance of fingerprints is determined solely by the receiving agency

• Agrees to the terms contained within this Disclosure and Consent Agreement.

ACKNOWLEDGEMENT
Receipt of fingerprinting services constitutes acknowledgment and acceptance of this Fingerprinting Disclosure and Consent Agreement.
