1. Valid Government-Issued Photo ID is required for identity verification

Acceptable IDs include:

- Driver’s License

- State ID Card

- Passport

- Military ID

- Permanent Resident Card (Green Card)

NOTE: The ID must be valid and not expired or tampered/altered

2. Fingerprint Card or Agency Form(s) [if provided]

Some agencies send applicants a fingerprint card and/or paperwork.
Examples:
- FD-258 Fingerprint Card

- SF-87 Federal Fingerprint Card

- State licensing fingerprint forms -and/or- Agency instruction sheet

NOTE: If the agency already gave you a card and/or instruction sheet, please bring them with you. If not, they may reject your fingerprints!

3. ORI Number / Agency Information

Many background checks require the ORI (Originating Agency Identifier) or agency address. Customers MUST bring:
- ORI number

- Agency name

- Mailing address for completed fingerprint cards to ensure fingerprints are submitted to the correct agency.

NOTE: Not having these may have your prints rejected by the agency.

4. Payment For Services
Customers should be prepared with the accepted payment methods. Services must be paid for before the start of any services. Accepted forms of payment are:

- Visa

- Mastercard

- American Express

- Discover

- Apple Pay

- Google Pay

- Cash App

Note: The process will not start until payment is made in full.

5. Clean Hands
All Customers must:

- Wash/Dry hands before the appointment
- Avoid lotions, oils, and creams, etc., before fingerprinting
- Remove heavy dirt or grease from hands
- Clean fingers produce quality prints

Note: Open cuts, sores, blisters, scabs, infections, etc. must not be present during your appointment.
